Job summary

The Care Team Hospice in Peoria, IL, seeks a compassionate Hospice RN Case Manager to coordinate patient care, develop individualized plans, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort and quality of life for patients at home.

This full-time role offers Monday–Friday hours, on-call opportunities with additional compensation, and a supportive culture with benefits including medical, dental, and vision insurance, 401(k) with company match, tuition

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.

Schedule & Work-Life Balance

• Monday-Friday, 8:00 AM - 4:30 PM
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only
