Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager in Peoria, IL. You will coordinate patient care, develop individualized plans of care, and collaborate with physicians and the interdisciplinary team to ensure comfort and quality of life for patients at the end of life.

Monday–Friday, 8:00 AM–4:30 PM with on-call opportunities and compensation. Prior hospice nursing experience and HCHB proficiency are preferred.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete visits, admissions, and interventions with accurate documentation in HCHB.
• Respond promptly to patient, family, and caregiver needs, including on-call situations.
• Coordinate care with physicians and interdisciplinary team to support outcomes.

Key Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
