Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Canton, OH. You will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life.

Qualifications

• Active RN license is required.
• Hospice nursing experience is preferred.
• Experience with Homecare Homebase (HCHB) is preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions with accurate documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support positive outcomes.
• Educate patients and families on symptom management, disease progression, medications, and end-of-life care.
