AdvantageCare Physicians (ACPNY) is seeking an experienced Registered Nurse to provide professional nursing care, support patient safety and quality of care, and act as a liaison between patients, staff and physicians. The role emphasizes collaboration within the Advanced Care Model.

The position includes clinical supervision of LPNs and Care Team Associates II, coordinating daily huddles, reviewing charts, and supporting patient education and chronic disease management. NYS RN license required.

Qualifications

• Graduate of an accredited school of nursing.
• Current NYS RN license in good standing.
• BLS certification; ACLS for GI/Cardiology when required.
• Experience in ambulatory or hospital settings preferred.

Responsibilities

• Provide professional nursing care and ensure patient safety and quality of care.
• Serve as liaison between patients, staff and physicians; support Advanced Care Model.
• Supervise specialty care team, delegate tasks, mentor staff, and assist with scheduling.
• Coordinate daily huddles, review patient charts, and manage supplies.
• Educate patients on self-management and chronic conditions; document notes in EHR.
