Qualifications

• Active, unrestricted RN licensure in the state of residence.
• BLS/CPR certified and able to perform standard nursing procedures.
• Strong communication in English, written and spoken.
• Experience with electronic medical records and patient confidentiality.
• Ability to assess patients without in-person interaction and coordinate care.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage.
• Execute protocol-driven chronic condition management and medication administration.
• Provide education to patients and caregivers on chronic conditions and preventive care.
• Perform onsite testing (point-of-care labs, EKGs) and manage care transitions.
