Job summary

Ascension Health is seeking an experienced registered nurse to join the Adolescent Psychiatry team at St. Elizabeth Hospital in Appleton, WI.

This part-time role offers two 12-hour shifts per week with weekend and holiday rotation, focusing on individualized patient care plans and careful documentation to support recovery. You will coordinate multidisciplinary care, follow physician and nursing orders, and assist with safe discharge planning to ensure a seamless transition across the continuum

Qualifications

• RN license required from Wisconsin Board of Nursing
• BLS certification within 30 days of hire
• Diploma from an accredited school/college of nursing or equivalent licensure

Responsibilities

• Lead the execution and monitoring of individualized patient care plans for optimal recovery and outcomes
• Document the patient care journey with meticulous detail and communicate critical updates to the interdisciplinary team
• Coordinate all clinical and therapeutic disciplines for a seamless patient-centered experience from admission to discharge
• Execute physician and nursing orders promptly to maintain patient safety and treatment adherence
• Assess discharge needs early and collaborate on comprehensive transition plans for safe continuum of care

• Schedule: Part-time days 24 hours per week | two 12-hour shifts per week 645am - 715pm | weekend and holiday rotation

• Lead the execution and continuous monitoring of individualized patient care plans to ensure optimal recovery progress and high-quality outcomes.
• Document the patient care journey with meticulous detail, recording conditions and communicating critical updates to the interdisciplinary team for timely intervention.
• Coordinate all clinical and therapeutic disciplines to ensure a seamless, cohesive, and patient-centered experience from admission through discharge.
• Execute physician and nursing orders promptly and precisely to maintain the highest standard of patient safety and treatment adherence.
• Assess patient discharge needs early and collaborate with the care team to develop comprehensive transition plans for a safe and successful continuum of care.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
