Qualifications

• Registered Nurse licensed in the state required.
• Experience administering medications.
• Knowledge of PCC EMR system.
• Familiarity with memory care protocols.

Responsibilities

• Assess resident health needs and develop nursing care plans.
• Administer medications and treatments as prescribed.
• Monitor progress and respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and families about health conditions and plans.
• Maintain accurate records in compliance with HIPAA and regulations.
• Support case management for continuity of care.
• Assist in specialized areas such as PT, OT, SUD, Pain management.]
