Job summary

Ascension Living HOPE in Wichita, KS seeks a full-time Registered Nurse for day shift in the Clinic. You will provide direct participant care including triage, blood draws and wound care, and participate in the IDT plan of care with potential after-hours/on-call rotation.

The role requires BLS and RN licensure, with a Diploma in nursing. Travel to participants’ residences and facilities may be required, offering growth and a supportive team environment.

Qualifications

• RN license required prior to hire date; BLS within 30 days of hire.
• Diploma from an accredited nursing program.
• Ability to educate participants and families as needed.

Responsibilities

• Provide direct participant care including acute triage, blood draws and wound care. Assist the care team for provision of participant care including after hours/weekend on-call rotation.
• Participate in development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care and update changes as necessary.
• Attend Interdisciplinary Team (IDT) care plan meetings and Participant/Family meetings as applicable. May perform assessments in a variety of settings including PACE Clinic and/or Adult Day Center.
• Develop and provide education to participants, caregivers, and associates as necessary including end of life discussions. May require travel to participant's place of residence, long term care and assisted living facilities.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

Required Credential(s):

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Registered Nurse obtained prior to hire date or job transfer date.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
