Job summary

Utopia Home Care, Inc. is seeking experienced Registered Nurses (RN) to join our Waterbury team for full-time, part-time, or per diem in-home visits. A $3,000 sign-on bonus is offered after 6 months of employment for eligible full-time staff.

We are a Medicare-certified, state-licensed home care agency serving CT since 1983, with a focus on comprehensive nursing, therapy, and home health aide services. Competitive pay, benefits, and a supportive onboarding are provided.

Qualifications

• Must hold RN or BSN and provide original certification.
• Must provide CT Driver's License or CT State ID and original Social Security Card.
• Must provide documentation of a physical and PPD within the past 12 months.

Responsibilities

• Perform health assessments and coordinate with physicians and care teams.

We have an assigned nurse educator who provides training as part of the on-boarding process.Registered Nurse (RN) Job Duties: Registered Nurses (RN) Case Managers oversee all medical services our clients receive which includes nursing, physical / occupational therapy, and home health aides. This begins with a comprehensive clinical assessment and creation of a specialized care plan, consistent with recommendations from the hospital, skilled nursing facility, and primary care physician. Direct nursing services include periodic health assessments, teaching, skilled intervention, care coordination, and medication management. All of this takes place in the client's home!Utopia provides a full spectrum of nursing services such as Medical / Surgical, Wound Care, Orthopedic Care, and Behavioral Health.
