Utopia Home Care, Inc. is seeking an experienced Registered Nurse (RN) to join our Hartford team in CT. This full-time, in-home nursing role offers $40.00–$45.00 per hour, with a $3,000 sign-on bonus after 6 months.

Responsibilities include comprehensive clinical assessments, care planning, medication management, and coordination with hospitals and primary care providers. Per diem and full-time options available, with generous benefits and travel reimbursement.

Qualifications

• Current RN license in good standing.
• Must provide original nursing certification documents.
• Background check and fingerprinting required within CT.
• Physical exam and PPD within past 12 months or willing to complete quickly.

Responsibilities

• Conduct comprehensive clinical assessments and develop individualized care plans.
• Provide direct nursing care, including assessments, teaching, medications, and care coordination.
• Coordinate with hospital, SNF, PCP, and home health aides to ensure continuity of care.
• Work in clients’ homes delivering compassionate, patient-focused nursing services.
