Job summary

Utopia Home Care, Inc. is seeking an experienced Behavioral Health Registered Nurse (RN) to join our Connecticut team on a per diem basis.

The RN will provide in-home behavioral health nursing services, create care plans, and coordinate with hospitals and primary care physicians to support seniors and clients with disabilities. Requirements include an RN or BSN, CT license, ability to provide essential documents (license, SSN, ID), and the capacity to complete a quick physical/PPD.

Qualifications

• RN or BSN certification from state-approved provider.
• Must provide original certification documents.
• CT driver’s license or CT state ID and original Social Security Card required.
• Must provide recent physical and PPD or be willing to obtain quickly.
• Background check and fingerprinting required.

Responsibilities

• Provide in-home nursing care and case management to clients.
• Perform health assessments, teaching, and medication management.
• Coordinate with hospital, facility, and primary care physicians to develop care plans.
• Deliver compassionate Behavioral Health services in clients' homes.

This begins with a comprehensive clinical assessment and creation of a specialized care plan, consistent with recommendations from the hospital, skilled nursing facility, and primary care physician. Direct nursing services include periodic health assessments, teaching, skilled intervention, care coordination, and medication management.
