Job summary

Utopia Home Care is seeking a per diem Registered Nurse (RN) to join our team in Bridgeport, CT. The RN will conduct clinical assessments, develop and implement care plans, and deliver direct nursing care in patients’ homes using a patient-centered approach.

Requirements include RN or BSN certification, CT driver’s license/ID, and the ability to obtain a physical and TB test promptly. Bilingual English/Spanish and home care experience are valued assets.

Qualifications

• RN or BSN certification from a state-approved program.
• At least 1 year of home care experience preferred.
• Bi-lingual English/Spanish skills considered a plus.

Responsibilities

• RN Case Managers oversee medical services and care plans in the client’s home.
• Deliver direct nursing services including assessments, interventions, and med management.
• Provide patient teaching and coordinate with other care providers.
