Utopia Home Care in Meriden, CT is seeking an experienced per diem Licensed Practical Nurse (LPN) to join our growing team for the Meriden branch. We have cases during the evening and weekend!

As an LPN, you will provide direct nursing services in clients' homes, conduct health assessments, coordinate care, and manage medications. Background checks, fingerprinting, and recent physical/PPD are required, with documentation of CT Driver's License/SSN also needed.

Qualifications

• Must have an LPN certification and possess the original document of certification from a state approved training provider.
• Must be able to provide original documents for CT Driver's License or CT State ID and original Social Security Card.
• Must be able to provide documentation of a physical and PPD taken within the past 12 months or be willing to obtain a physical and PPD test quickly.

Responsibilities

• Provide direct nursing services in the client's home, including health assessments and medication management.
• Perform skilled interventions, care coordination, and patient teaching.
• Support seniors and clients with disabilities with patience and compassion.
