Job summary

Immediate Home Health and Hospice in Philadelphia, PA is seeking an experienced Registered Nurse to provide in-home care to patients. You will collaborate with physicians, nurses, therapists, social workers and families to develop and implement patient-centered care plans.

This position offers flexible hours, full-time or part-time options, weekly pay, and a comprehensive benefits package including medical, dental, vision and life insurance, plus PTO and a 401(k) match.

Qualifications

• Active RN license valid in PA.
• Two or more years of nursing experience.
• Nursing supervisory/leadership experience preferred.

Responsibilities

• Deliver comprehensive in-home nursing care in patients' homes.
• Assess, plan, direct, and evaluate patient care plans.
• Manage emergencies promptly and professionally.
• Complete medication administration and treatment needs for assigned patients.
• Provide nursing care per regulations and company policy.
• Prepare and maintain progress notes in a timely manner.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Active RN state license
• Valid drivers’ license
• Two or more years of applicable nursing experience
• Nursing supervisory/leadership experience (preferred)
