Job summary

Immediate Home Health & Hospice in the Philadelphia area is seeking a registered nurse to join our home-based care team in Montgomery County. You will provide comprehensive nursing care to patients in their homes and collaborate with physicians, therapists, and caregivers.

This role offers flexible hours (full-time or part-time) and a supportive, collaborative environment with weekly pay and a robust EMR system.

Qualifications

• Active RN state license.
• Two or more years of applicable nursing experience.
• Home health experience preferred.
• Valid driver's license.

Responsibilities

• Provide care to patients in their homes in coordination with physicians, nurses, therapists, social workers, and caregivers.
• Assess, plan, direct, and evaluate each patient’s care plan to meet needs.
• Manage emergency situations promptly and professionally.
• Complete medication administration and treatments for assigned patients.
• Provide nursing care in accordance with regulations and company policy.
• Prepare and maintain accurate clinical and progress notes.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Two or more years of applicable nursing experience
• Home Health experience (preferred)
