Job summary

Immediate Home Health & Hospice in Philadelphia seeks a Registered Nurse to provide in-home patient care in the North Philadelphia area. The role involves coordinating with physicians, therapists, and caregivers to deliver comprehensive, patient-centered services in patients' homes.

The ideal candidate holds an active RN license, has at least two years of nursing experience, and prefers home health settings. Flexible scheduling and strong patient advocacy are emphasized in this role.

Responsibilities

• Deliver comprehensive nursing care to patients in their homes in the North Philadelphia service area.
• Assess, plan, direct, and evaluate each patient’s care plan.

• Complete medication administration and treatment needs for all assigned patients.
• Provide nursing care in accordance with regulations and company policy.
• Prepare and maintain accurate clinical and progress notes.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Active RN state license
• Valid drivers’ license
• Two or more years of applicable nursing experience
• Home Health experience (preferred)
