Job summary

Immediate Home Health and Hospice is seeking an experienced registered nurse to provide in-home health and hospice care in the Philadelphia area. You will collaborate with physicians, NPs, nurses, therapists, social workers, and caregivers to coordinate patient-centered care during home visits.

The role requires an active RN license, a valid driver’s license, and 2+ years of nursing experience. Supervisory experience is preferred.

Qualifications

• Active RN license in good standing.
• Valid driver's license required.
• 2+ years of nursing experience in home health or related setting.
• Nursing supervisory/leadership experience preferred.

Responsibilities

• Deliver comprehensive and coordinated in-home nursing care to patients.
• Assess, plan, implement, and evaluate each patient’s care plan.
• Respond promptly to emergencies with professional judgment.
• Administer medications and treatments as prescribed.
• Provide nursing care in accordance with regulations and company policy.
• Prepare accurate clinical progress notes in a timely manner.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Active RN state license
• Valid drivers’ license
• Two or more years of applicable nursing experience
• Nursing supervisory/leadership experience (preferred)
