Job summary

CareRing Health in the Philadelphia area seeks a dedicated Registered Nurse to provide in-home health and hospice services across Montgomery County. You will collaborate with physicians, nurses, therapists, and social workers to deliver coordinated patient care during home visits.

The role requires active RN licensure, a valid driver’s license, and 2+ years of nursing experience, with home health experience preferred. We offer flexible hours and a supportive team environment.

Qualifications

• Active RN state license.
• Valid drivers' license.
• Two or more years of applicable nursing experience.
• Home Health experience (preferred).

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to patients in their homes – conduct patient home visits.
• Assess, plan, direct, and evaluate each patient’s care plan to meet the patient’s needs.
• Manage emergency situations in a prompt, precise, and professional manner.
• Complete medication administration and treatment needs for all assigned patients.
• Provide nursing care in accordance with applicable regulations and Company policy.
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
