Job summary

Immediate Home Health and Hospice is actively seeking qualified registered nurses to provide in-home health and hospice services in the Philadelphia area. You will collaborate with physicians, nurses, therapists, and social workers to deliver coordinated patient care in patients' homes.

The role involves assessing and directing care plans, managing emergencies, and documenting progress notes. A valid RN license and driver's license are required; two+ years of nursing experience and leadership

Qualifications

• Active RN license required in the state of job location.
• Valid driver’s license and reliable transportation.
• Two or more years of nursing experience in home health, hospice, or related settings.
• Nursing supervisory/leadership experience preferred.

Responsibilities

• Collaborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive home-based care; conduct patient home visits.
• Assess, plan, direct, and evaluate each patient’s care plan to meet individual needs.
• Manage emergencies promptly and professionally.
• Complete medication administration and treatment needs for all assigned patients.
• Provide nursing care in accordance with regulations and company policy.
• Prepare and maintain accurate clinical and progress notes in a timely manner.

Responsibilities

What You Will Do:

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

Qualifications:

• Active RN state license

• Two or more years of applicable nursing experience
• Nursing supervisory/leadership experience (preferred)
