Job summary

Immediate Home Health & Hospice is seeking an experienced RN to deliver in-home health care across the Montgomery County service area in the Philadelphia region. You will work collaboratively with a multi-disciplinary team to assess and manage patient care during home visits.

The position requires an active RN license, a valid driver's license, and a minimum of two years of nursing experience; home health experience is preferred.

Qualifications

• Active RN state license.
• Two or more years of nursing experience.
• Home Health experience preferred.

Responsibilities

• Collaborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to provide in-home patient care and conduct visits.
• Assess, plan, direct, and evaluate each patient’s care plan to meet needs.
• Manage emergencies promptly, precisely, and professionally.
• Complete medication administration and treatment needs for all assigned patients.
• Provide nursing care following applicable regulations and company policy.
• Prepare and maintain accurate clinical and progress notes on a timely basis.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Two or more years of applicable nursing experience
• Home Health experience (preferred)
