Job summary

Immediate Home Health and Hospice in Philadelphia is seeking a skilled registered nurse to provide in-home care to patients, coordinating with physicians, therapists, and caregivers.

Qualifications

Responsibilities

• Deliver comprehensive and coordinated in-home patient care with physicians, nurses, therapists, social workers, and caregivers.
• Assess, plan, direct, and evaluate each patient’s care plan to meet needs.
• Manage emergency situations promptly and professionally.
• Complete medication administration and treatment needs for all assigned patients.
• Provide nursing care in accordance with regulations and policy.
• Prepare and maintain accurate clinical and progress notes.

Responsibilities

What You Will Do:

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

Qualifications:

• Active RN state license
• Valid drivers’ license
• Two or more years of applicable nursing experience
• Nursing supervisory/leadership experience (preferred)
