Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate patient home care, perform admission visits, and determine eligibility for services. You will collaborate with medical teams to develop and follow the plan of care in the patient's home.

Qualifications include an RN license, at least one year of RN experience (two preferred), CPR and hepatitis paperwork, and a valid driver’s license with reliable transportation.

Qualifications

• Graduate of an accredited nursing program (Diploma/Associate/Bachelor).
• Current state RN license.
• Valid driver's license.
• One year of RN experience; two years preferred.
• CPR and Hepatitis consent/declination.
• Reliable transportation.

Responsibilities

• Coordinate all aspects of home care for the patient.
• Perform the admission visit in the patient’s home.
• Determine eligibility for home care services.
• Develop and follow the plan of care with interdisciplinary teams.
