Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of patient home care. You will perform admission visits, determine eligibility for home care services, and develop the plan of care to be followed.

You will interact with other medical disciplines and provide professional care in the patient’s home, with a focus on quality outcomes and patient comfort. Qualified candidates hold an RN license, a valid driver’s license, CPR proof, and at least one

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis vaccination/declination.

Responsibilities

• Coordinate all aspects of the patient’s home care.
