Job summary

Angels Care Home Health in Vincennes, IN is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will conduct admission visits, determine eligibility for home care services, and develop the plan of care while collaborating with other medical professionals.

Requires RN license, valid driver’s license, CPR, and 1 year of RN experience; we offer strong benefits and professional development opportunities.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse (RN).
• Current state Driver’s License; CPR certification required.
• One year of RN experience; two years preferred.
• Reliable transportation with valid auto liability insurance.
