Job summary

Angels Care Home Health in Texas seeks an experienced Registered Nurse (RN) Case Manager to coordinate patient home care, perform admission visits, and assess eligibility for services.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.
