Job summary

Angels Care Home Health is seeking an experienced Registered Nurse Case Manager to coordinate all aspects of patient home care. You will collaborate with medical teams, conduct admission visits, assess eligibility for home health services, and develop the plan of care to be followed.

The role requires an active RN license, a valid driver’s license, CPR certification, and reliable transportation. We offer strong benefits, PTO, training, and opportunities for advancement.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• One-year experience as a Registered Nurse; two years preferred.
• Proof of CPR and Hepatitis vaccination consent/declination.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform admission visit to determine eligibility for home care services.
• Develop and follow the plan of care with input from the patient and care team.
