Job summary

Angels Care Home Health in Lyons, KS is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will perform the admission visit in the home, determine eligibility for home health services, and develop the plan of care to be followed, collaborating with other medical disciplines as needed.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.

• Develop and monitor the plan of care to be followed.
