Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate comprehensive home health care for patients. You will perform admission visits, assess eligibility for home care, and develop individualized care plans in collaboration with medical teams.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care to the patient.
• Perform the admission visit to the patient in the home, determine eligibility for home care services, and develop the plan of care to be followed.

Education

Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing

Current state Driver's License

CPR certification and Hepatitis consent/declination

Reliable transportation with auto liability insurance
