Job summary

Angels Care Home Health is seeking an Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will interact with other medical disciplines as needed and provide professional care to patients in their homes.

You will also perform admission visits to determine eligibility for home care services and develop the plan of care to be followed. A valid RN license, CPR, and reliable transportation are required, with a driver’s license and insurance.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• One-year experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.
