Job summary

UnitedHealth Group is seeking a Registered Nurse for Home Health in Gallipolis, OH. This full-time role involves coordinating with physicians and healthcare teams to deliver comprehensive home-based care and ensure continuity of care for patients at home.

The position requires an active RN license in the state, at least 1 year of home health experience, and a valid driver's license with reliable transportation. CPR certification is preferred or obtainable within 90 days of hire.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience preferred.
• Must have a valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire preferred.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Provide high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements, syncing timely per LHC policy.
• Make initial and/or comprehensive nursing evaluation visits to ensure patients meet home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.
