UnitedHealth Group is seeking a Registered Nurse for Home Health in Liberty, KY. This role provides and directs nursing care in patients' homes, coordinating with physicians and health care teams to ensure quality care and compliance with laws and regulations.

The position offers a full-time schedule, a pay range of $67,905 to $101,857 total cash target per year, and opportunities to supervise home health aides and other nurses, with on-call rotations as needed.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of Home Health experience preferred.
• Current Driver's License required.
• Current CPR Certification preferred or ability to complete within 90 days of hire.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other health care professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses.
• Provide required supervisory visits.
• Provide high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.
