Job summary

UnitedHealth Group is seeking a Home Health Registered Nurse to provide and direct nursing care to patients in their homes in Clinton, MS. You will coordinate with physicians and healthcare professionals to ensure continuity of care and patient-family connections.

The role involves developing plans of care, supervising home health staff, performing OASIS assessments, and ensuring eligibility and medical necessity guidelines are met, while staying aligned with payer requirements and agency

Qualifications

• Current and unrestricted RN licensure in Mississippi.
• 1+ years Home Health experience preferred.
• CPR certification preferred or ability to complete within 90 days of hire.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements, syncing timely per LHC policy.
• Ensure patients meet home health eligibility and medical necessity guidelines.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.
• Adhere to and participate in the agency's utilization management model.

Requirements

Education

• Current and unrestricted RN licensure in State of Mississippi

Experience

• 1+ years of Home Health experience preferred

Licenses & Certifications

• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Current CPR Certification or ability to complete within 90 days of hire preferred
