Job summary

Geisinger Home Health is seeking a Registered Nurse to provide and direct nursing care in patients' homes across a flexible schedule. You will coordinate with physicians and the care team to ensure continuity of care and high-quality patient outcomes.

The role includes supervising home health aides, performing supervisory visits, and completing OASIS assessments while ensuring infection-control standards. A valid RN license and ability to drive are required, with a sign-on bonus for full-time

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ year of Home Health experience preferred.
• Cleans and safe driving record; valid driver's license and vehicle.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Initiate and revise the plan of care with physician and team.
• Supervise home health aides and licensed nurses; provide instruction.
• Perform required supervisory visits.
• Deliver high-quality clinical services per scope and infection control.
• Complete comprehensive OASIS assessments and reconcile meds.
• Document visits per policy and payer requirements.
• Assess home health eligibility via nursing visits.
• Communicate with agency staff promptly and clearly.
• Orient new staff and act as a preceptor; participate in on-call rotations.

About This Role

As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes, coordinating with healthcare professionals to ensure continuity of care. This role is ideal for those who are dedicated to delivering high-quality patient care in a flexible environment.

• Pay: $71,677 - $107,516 annual total cash target pay

• Sign-On Bonus: $5,000 Sign-on Bonus available for full-time hires
• Service Area: Covers Mifflin County, Juniata County, and Parts of Snyder County

What You'll Do

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Provide high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements, syncing timely per LHC policy.
• Make initial and/or comprehensive nursing evaluation visits to ensure patients meet home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.

Requirements

Education:

• Current and unrestricted RN licensure in state of practice

Experience:

• 1+ years of Home Health experience preferred

Licenses & Certifications:

• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Current CPR Certification or ability to complete within 90 days of hire preferred
