Job summary

Senior Community Care of Kentucky PACE (Northern) in Florence, KY is seeking a Registered Dietitian to support nutrition care, menu planning, counseling and therapeutic diets within our PACE program. You will be an active member of the IDT, helping manage complex medical and social needs of older adults.

Qualifications

• Current registration as a Registered Dietitian (RD/RDN) eligible to practice in Kentucky.
• Strong communication and customer service skills.
• Ability to follow care plans and orders approved by the IDT.
• Must have medical clearance for communicable diseases and up-to-date immunizations.
• Must clear background check.
• Must have a valid driver’s license and transportation.

Responsibilities

• Assess nutritional needs and develop nutritional care plans.
• Work as a member of the Interdisciplinary Team (IDT) and monitor participants.
• Provide nutritional counseling and education for program participants.
• Oversee and manage the dining program from menu planning to helping serve meals.
