Job summary

Legacy Healthcare is seeking a Registered Dietitian to join a PACE program in the Greater Louisville area. The role involves comprehensive nutritional assessments, care plan development, and coordination with an interdisciplinary team to support frail seniors wishing to stay in their communities.

The RD will provide nutrition counseling, monitor progress, and ensure CMS and state guidelines are followed. Indiana licensure is required; Master’s is preferred.

Qualifications

• Bachelor's in Nutrition, Dietetics, or related field (Master’s preferred).
• Current RD/RDN credential through CDR.
• Licensed Dietitian/Nutritionist in Indiana or ability to obtain upon hire.
• 1–2 years experience with elderly or chronically ill populations (PACE/geriatric preferred).
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Conduct nutritional assessments for PACE participants.
• Develop and implement individualized nutrition care plans.
• Collaborate with physicians, nurses, social workers, and other team members.
• Provide nutrition counseling and education to participants, caregivers, staff.
• Monitor progress and adjust plans as needed.
• Ensure compliance with CMS and PACE guidelines.
