Job summary

Hospice of The Bluegrass, Inc. is seeking a Registered Dietitian for the Care Guide Partners PACE program in Elizabethtown, KY. The role focuses on nutrition assessment and education for seniors who require nursing-level care at home.

You will screen nutritional status, plan interventions, and collaborate with the Interdisciplinary Team to ensure meals meet participant needs. Travel within the service area and CPR certification are required.

Qualifications

• Licensed Registered Dietitian in Kentucky.
• 1 year experience with frail/elderly population or training on hire.
• CPR certification required.
• Willingness to travel within service area.
• Proficient with technology-based tools including EMR.
• Independent and team player with good communication skills.
• Strong organizational skills and ability to multitask.
• Knowledge of healthcare, managed care, Medicare/Medicaid regulations.

Responsibilities

Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home.

Schedule: Monday-Friday 8:30-5:00pm

Position Summary:

The Registered Dietitian screens and assesses participant nutritional status. Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. Performs nutritional assessment and provides nutritional education for the participants requiring interventions. Coordinates serving of meals; monitors/coordinates kitchen facilities to ensure standards are met. Coordinates with contracted meal services to ensure meals meet the needs of the PACE participants. Provides education to other health care professionals as appropriate.

Position Qualifications:

• Currently licensed Registered Dietitian in the State of Kentucky.
• Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
• Must maintain CPR certification.
• Must have flexibility to travel within the service area.
• Proficiency with technology-based tools, including electronic medical record.
• Able to work independently and as an effective member of a team.
• Excellent communication skills, with ability to communicate with internal stakeholders at all levels of the organization and external stakeholders from varied backgrounds.
• Excellent organizational skills; able to multitask and maintain numerous initiatives with time sensitivity.
• Working knowledge of principles and practices of healthcare, healthcare systems, managed care, Medicare and Medicaid benefits and regulations.
• Knowledge of safety and infection control requirements for healthcare facilities.
