Job summary

Legacy Healthcare in Louisville, KY is seeking a Registered Dietitian to join our PACE program team. You will conduct nutritional assessments, develop care plans, and provide counseling to frail seniors who wish to stay in their communities.

This direct hire role requires RD credentials, active state licensure or ability to obtain, and 1–2 years experience with elderly or chronically ill populations. Collaboration with physicians, nurses, and social workers is essential, along with attention to

Qualifications

• Bachelor's degree in Nutrition, Dietetics, or related field (Master’s preferred).
• Current RD/RDN credential through CDR (Commission on Dietetic Registration).
• Licensed Dietitian/Nutritionist in the state of Indiana (or ability to obtain upon hire).
• Minimum of 1–2 years of experience working with elderly or chronically ill populations—PACE, long-term care, home health, or geriatric settings preferred.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a collaborative team.

Responsibilities

• Conduct comprehensive nutritional assessments for PACE participants.
• Develop and implement individualized nutrition care plans based on participant needs, medical diagnoses, and cultural preferences.
• Collaborate with physicians, nurses, social workers, and other interdisciplinary team members to coordinate care.
• Provide nutrition counseling and education to participants, caregivers, and staff.
• Monitor participant progress and adjust nutrition plans as needed.
• Ensure compliance with regulatory and PACE-specific guidelines (CMS, state, etc.).
• Support food service operations and menu planning in conjunction with kitchen staff (if applicable).
• Participate in team meetings, quality improvement activities, and staff training.
