Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to join our team in San Leandro, CA for 16 hours per week, serving our long-term care community.

You will perform clinical nutrition assessments, document using the Nutrition Care Process in the EMR, participate with the interdisciplinary care plan team, complete MDS - Section K, calculate TF, and make recommendations for wounds and significant weight changes.

Qualifications

• Must be credentialed by the CDR.
• Must hold active dietetic licensure.
• Positive attitude and client-centered approach.
• Strong communication skills and collaboration with care teams.

Responsibilities
