Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to join our team in Oakland, California. This opportunity serves our long-term care community located in San Leandro, CA for 16 hours a week.

The ideal candidate will complete clinical nutrition assessments, document in the EMR system, and collaborate with the interdisciplinary care plan team. Requirements include being credentialed by the CDR and holding dietetic licensure.

Qualifications

• Must have dietetic licensure and be credentialed by the CDR.
• Possess good communication skills and a positive attitude.
• Must be client-centered and able to work in a team.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
