Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to join our team for a 16 hours/week position serving our long-term care community in Carmichael, CA. Flexible schedule and excellent pay are offered.

Weekend coverage is allowed, and drive time is offered to support care coverage. Opportunities to contribute to wound care and weight-change management are part of the role.

Qualifications

• CDR credentialing required.
• Licensed dietitian with active licensure.
• Demonstrated strong communication skills in clinical settings.
• Patient-centered approach to care and collaboration.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR.
• Collaborate with interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF (aimed at wound and weight change management).
• Provide evidence-based nutrition recommendations.

• Completing clinical nutrition assessments
• Documenting using the Nutrition Care Process in the EMR system
• Collaborating with the interdisciplinary care plan team
• Completing the MDS - Section K
• Calculating TF
• Making recommendations for wounds and significant wt changes

• Credentialed by the CDR
• Dietetic licensure
