Job summary

Nutrition that Works, LLC in Merced, CA is seeking a consultant dietitian to provide care for our long-term care community on a 16 hours/week schedule. The role features a flexible schedule and competitive pay in a patient-centered environment.

You will perform clinical nutrition assessments, document using the Nutrition Care Process in the EMR, and collaborate with the interdisciplinary care plan team. MDS - Section K, TF calculations, and wound/weight-change recommendations are also part of

Qualifications

• CDR-credentialed dietitian with state licensure.
• Strong communication and client-centered approach.
• Experience documenting using Nutrition Care Process in EMR.
• Familiar with long-term care nutrition needs.
• Positive, collaborative team member.

Responsibilities

• Complete clinical nutrition assessments.
• Document care plans using the Nutrition Care Process in the EMR system.
• Collaborate with interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF as part of care planning.
• Make wound and weight-change recommendations.
