Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to join our team for a 24-hour-per-week opportunity in Montebello, CA. This role supports our long-term care community with flexible scheduling and competitive pay.

Responsibilities include completing nutrition assessments, documenting with the Nutrition Care Process in the EMR, collaborating with the care plan team, and contributing to wound and weight-change management. LTC experience is preferred.

Qualifications

• CDR credentialing and dietetics licensure required or in good standing.
• Experience in long-term care is preferred.
• Strong communication and patient-centered approach.
• Ability to document using the Nutrition Care Process in EMR.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Complete MDS - Section K and calculate TF.
• Make recommendations for wounds and significant weight changes.
