Job summary

APHealth is seeking a Regional Program Coordinator to lead ECM, CS, and CHW services in Northern California. You will supervise Care Managers, ensure program quality, and maintain a small caseload while traveling across the region.

The role requires a bachelor’s degree in a related field, 5+ years in care management, and 3+ years of supervision, with strong leadership and communication skills and knowledge of CalAIM and HIPAA. This is field-facing leadership for operational excellence.

Qualifications

• Bachelor’s degree in a related field and five+ years in care management or related roles.
• Three+ years of supervisory or leadership experience.
• Knowledge of CalAIM, ECM, and Community Supports programs.
• Strong leadership, communication, organization, and problem-solving skills.
• Experience with vulnerable populations and complex needs.
• Proficiency with electronic health records and reporting systems.

Responsibilities

• Supervise, coach, mentor, and support Care Managers.
• Conduct performance reviews and staff development.
• Monitor productivity, documentation quality, member engagement, and outcomes.
• Lead team meetings, case reviews, and quality improvement activities.
• Coordinate day-to-day regional operations for ECM, CS, and CHW programs.
• Ensure compliance with CalAIM, DHCS, HIPAA, contracts, and APHealth requirements.
• Maintain a direct caseload of approximately 20 members.
• Build relationships with plans, providers, and community partners.
