Job summary

The April Parker Foundation is seeking a Regional Program Coordinator to lead Enhanced Care Management, Community Supports, and Community Health Worker services in Northern California. You will supervise Care Managers, manage performance, ensure compliance, and maintain a small caseload of about 20 members.

This field-based leadership role requires extensive regional travel, strong operations oversight, and a commitment to high-quality, outcome-driven care in a dynamic environment.

Qualifications

• Bachelor’s degree in Social Work, Public Health, Human Services, Psychology, Nursing, or related field.
• At least five years of experience in care management, healthcare, behavioral health, case management, or social services.
• At least three years of supervisory, lead, or management experience.
• Knowledge of CalAIM, ECM, and CS programs.
• Strong leadership, communication, organization, and problem-solving skills.
• Experience working with vulnerable populations or individuals with complex needs.
• Proficiency with electronic health records, care management platforms, and reporting systems is ideal.

Responsibilities

• Supervise, coach, mentor, and support Care Managers.
• Conduct performance reviews and provide ongoing staff development.
• Monitor productivity, documentation quality, member engagement, and outcomes.
• Lead team meetings, case reviews, and quality improvement activities.
• Support recruitment, onboarding, training, and field coverage.
• Coordinate day-to-day regional operations for ECM, CS, and CHW programs.
• Ensure compliance with CalAIM, DHCS, HIPAA, and APHealth requirements.
• Monitor regional performance metrics, caseloads, outreach activity, documentation, and outcomes.
• Implement workflows, SOPs, and continuous improvement initiatives.
• Support audits, chart reviews, action plans, and funder reviews.
• Maintain a direct caseload of approximately 20 members.
• Complete outreach, assessments, care plans, care coordination, and case closure.
• Model best practices in engagement, service delivery, and documentation.
• Support service continuity during staffing shortages and coverage gaps.
• Represent APHealth, build partnerships, attend community events, and support growth.
