• Covering multiple programs in an assigned region to ensure appropriate coverage and high quality care
• Provide direct care for individuals with disabilities and support independent living
• Assist in training DSPs and ensure quality delivery of care
• Document health, incidents, and logs in required systems
• Support individuals with daily living activities per ISP/BSP/medical plans
• Treat individuals with dignity and respect and advocate for their preferences
• Assist with personal cares, hygiene, appointments, and medication administration
• Assist with toileting and continence care and observe changes in condition
• Assist with bathing and grooming per individual protocols
• Assist with meal preparation and dietary choices
• Foster positive relationships with individuals, guardians, staff and leadership
• Maintain thorough documentation across health tracking, incident reports, and logs
