SourcePro Search, LLC seeks a Clinical Manager to supervise patient care services and personnel in a home health setting. The role leads CHAP readiness, QAPI initiatives, and regulatory survey preparation while ensuring high-quality, compliant care across the organization.

The position requires an RN licensed in the operating state, with 5+ years of RN experience and 2+ years in supervision. Travel to multiple locations may be required.

Qualifikationen

• RN licensed and registered in the state of operation.
• Minimum 5 years RN experience with at least 2 years in supervision in Home Health.
• Experience leading RN services in home, community, or clinic settings.
• CPR certification maintained during clinical role.

Aufgaben

• Oversight of daily clinical and administrative operations, as primaryStaff contact.
• Ensure compliance with professional standards and state/federal regulations.
• Provide leadership and backup support in absence of Territory Administrator.
• Coordinate intake, referrals, and develop individualized care plans.
• Drive quality assessment and coordinate transfers/discharges with clinicians.
• Manage OASIS data and discharge processes; oversee discipline-only discharges.
• Lead QAPI initiatives and track incidents with corrective actions.
• Maintain accreditation readiness and HHCAHPS survey processes.
• Lead Emergency Preparedness Planning and ongoing monitoring.
