Job summary

Meritas Advisory Group, LLC seeks a Clinical Manager to oversee patient care services and staff, ensuring safe and effective delivery of care. The role leads CHAP and survey readiness, drives QAPI initiatives, and monitors patient satisfaction and complaint resolutions.

The position requires RN licensure, 5+ years RN experience with supervisory background in Home Health, and the ability to travel to all business locations. Strong leadership and regulatory compliance skills are essential.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as an RN.
• Minimum 5 years RN experience with at least 2 years in a supervisory role in Home Health.
• Experience managing RN services in the home, community, or clinic is preferred.
• Licensed to practice as an RN in the state of operation; CPR certification required.
• Must be able to travel to all business locations and maintain a valid driver's license.

Responsibilities

• Oversee Operations: Manage day-to-day clinical and administrative operations, serving as a primary contact for staff and patients.
• Ensure Regulatory Compliance: Direct care delivery to align with standards, policies, and regulations.
• Provide Leadership & Backup Support: Act as leader in absence of Territory Administrator and cover PCM and CSS roles as needed.
• Coordinate Patient Care: Manage intake and referrals with interdisciplinary team to create updated care plans.
• Drive Quality & Assessment: Evaluate care quality, assess patient needs, and collaborate on transfers and discharges.
• Manage OASIS & Discharge Data: Review discharge data, edit/lock OASIS forms, oversee discharge processes.
• Lead QAPI: Direct QAPI initiatives, track incidents, analyze trends, and implement corrective actions.
• Maintain Survey Readiness & Analytics: Maintain accreditation readiness, perform audits, and manage HHCAHPS surveys.
• Oversee Emergency Preparedness: Lead EPP activities, activation, reporting, and event monitoring.
