Job summary

Essenti Talent is seeking a Clinical Manager to oversee patient care services and clinical staff across home health operations. You will coordinate care, supervise providers, ensure safety and regulatory compliance, and drive CHAP readiness and QAPI initiatives.

You will lead discharge planning, OASIS data handling, and survey distribution, while maintaining high patient satisfaction and effective staff coaching. Travel to multiple locations is required.

Qualifications

• Graduate from an approved nursing program and licensed to practice as an RN in the operating state.
• 5 years RN experience with at least 2 years in Home Health supervision.
• Experience managing RN services in home, community, or clinic.
• Must hold CPR certification.
• Must be a licensed driver with insured vehicle and travel to all locations.

Responsibilities

• Oversee day-to-day clinical and administrative operations and serve as primary staff/patient contact.
• Ensure compliance with professional standards and regulations.
• Provide leadership and backup support during absence of Territory Administrator.
• Coordinate patient care and develop individualized care plans with interdisciplinary team.
• Drive QAPI initiatives and monitor quality, incidents, and CAHPS survey distributions.
• Maintain survey readiness and perform detailed audits.
• Lead Emergency Preparedness Planning (EPP) and post-activation reporting.
• Oversee OASIS and discharge data, including discharge forms.
