Job summary

Eventus WholeHealth is seeking a behavioral health support professional to assist residents in long-term care facilities under supervision by a licensed clinician. Regional travel will be required to support treatment plans and coordinate care with families and facility staff.

You will conduct assessments, educate patients and families, and maintain timely documentation as part of a collaborative behavioral health team.

Qualifications

Bachelor's degree in Counseling, Social Work, Psychology or related field.
Experience in a psychiatric or long term care setting preferred.
Ability and willingness to function independently and proactively in a long term care setting.

Responsibilities

Conduct assessments that include the behavioral and social needs of the patients.
Meet with families and facility staff to coordinate care as recommended by licensed professionals.
Assess the patient’s knowledge of their clinical condition and provide education and self-management support.
Provide outreach to clients who are having problems with treatment adherence.
Assist in the development of the individual goals and behavioral health treatment plan.
Coordinate with other agencies and community providers.
Maintain timely documentation of work.
Serve as an active member of the behavioral health treatment team.
