Qualifications

• Bachelor’s degree in Counseling, Social Work, Psychology or related field.
• Experience in a psychiatric or long term care setting preferred.
• Customer service experience preferred.
• Ability to work independently and proactively in a long term care setting.
• Proficient use of EMR and Microsoft Office tools (Teams, Excel).
• Strong written and verbal communication; ability to handle confidential information.

Responsibilities

• Conduct assessments covering behavioral and social needs of patients.
• Meet with families and facility staff to coordinate care.
• Educate patients on their clinical condition and self-management.
• Provide outreach to improve treatment adherence.
• Assist in developing individual goals and treatment plans.
• Coordinate with other agencies and community providers.
• Maintain timely documentation of work.
• Be an active member of the behavioral health treatment team.
